Leaving the ADF is a big step. Transition Medicals make sure you're supported both before and after you discharge, so there's no disruption to your healthcare.
Before you leave the ADF

Get set up for a smooth handover. You may be referred by your ADF Medical Officer through BUPA, or self-refer. A clinician will sit down with you to understand your medical and allied health needs, help complete treatments before discharge, and walk you through entitlements like PAMT, allied health support, household services and incapacity certificates.
We can also lodge provisional claims early so you're not starting from scratch.
After you discharge
~1 month after
Once you've left the ADF, the focus shifts to putting your care plan into action. Since ADF-issued referrals and prescriptions don't carry over, this consultation bridges the gap with new referrals and short-term prescriptions, PAMT activation where eligible, and a warm handover to a local GP. The goal: no gaps in your medical support.
